Before: what the trial patients started with
The entry criteria are the “before”. The pharmacokinetic study enrolled men with morning total testosterone under 350 ng/dL and LH under 12 IU/L, measured on at least two occasions. The phase III studies enrolled overweight men aged 18 to 60 with morning testosterone at or below 300 ng/dL and low or normal LH, measured twice, two to ten days apart. That combination, low testosterone with LH that is not elevated, is secondary hypogonadism: the testes can make testosterone, the signal telling them to is weak. It is the situation enclomiphene is designed for, and it is why a clinician orders baseline labs before prescribing it rather than starting from symptoms alone.
After six weeks: the pharmacokinetic study
Three doses, 6.25 mg, 12.5 mg and 25 mg once daily, were compared with a standard dose of testosterone gel. After six weeks of daily dosing, the mean morning total testosterone on 25 mg was 604 ng/dL, plus or minus 160, against 500 ng/dL, plus or minus 278, on the gel, a difference the authors did not find statistically significant. All three enclomiphene doses raised testosterone. The direction of LH was the difference that matters: it rose on enclomiphene and was suppressed on the gel. After sixteen weeks: the phase III studies
Two randomized, double-blind, placebo-controlled phase III studies compared two enclomiphene doses with testosterone gel over sixteen weeks. Testosterone increased in every treatment group. FSH and LH increased in the enclomiphene groups and decreased in the gel group. Sperm concentration stayed in the normal range on enclomiphene, while the gel group showed a marked reduction in sperm production. The earlier phase II trial had found the same pattern, with testosterone, estradiol and LH rising on enclomiphene to a similar degree as on gel, and sperm counts conserved.
What rose that you did not ask about
Estradiol. In the phase II trial it increased alongside testosterone, to a similar degree as on testosterone gel. That is expected from the mechanism, since more testosterone means more substrate for conversion to estradiol, and it is why estradiol sits on the follow-up lab panel. A “before and after” that only reports testosterone is missing the number a clinician watches most closely after the first weeks.
What the trials did not measure
Energy, libido, mood, body composition, strength and sleep were not endpoints in these studies. The forum before-and-afters are almost entirely about those, and almost none of them come with a baseline lab, a dose or a timeline. That does not mean nothing changes; it means no trial can be quoted for it, and any provider who quotes one is inventing. The honest version is: the bloodwork moves within weeks, the follow-up labs show whether it moved for you, and how you feel is tracked between you and your clinician.
Before and after on TRT, for comparison
Testosterone gel raised testosterone in the same trials, and the numbers looked similar at six weeks. The “after” diverged everywhere else: LH and FSH fell on gel and rose on enclomiphene, and sperm concentration fell on gel while enclomiphene kept it in the normal range.
